Dr. Hassler's Bio from SportzFactory.com:

The Sportz Factory has a combined experience of over 20 years in Sports Health Science and the Sports Performance industry.  

Leading the way is Dr. Erin Hassler, with almost 2 decades of hands-on experience at multiple levels of Sports Medicine and Sports Performance Enhancement.  

Credentials include but are not limited to the Advisory Board of Athletic Trainers, Board of Certification and National Academy of Sports Medicine Performance Enhancement Specialist.​

Since beginning her career as Head Athletic Trainer for Channelview Independent School District in 2000, she has enjoyed industry success as either a staff or contract Athletic Trainer for several organizations in Houston and surrounding areas.  

Dr. Hassler has practiced sports medicine in both public and private school systems the WNBA – Houston Comets and the US Olympic Committee. 

Now as a volunteer medical provider with USA Track and Field, she has served on international teams, including travel to Canada, South America, and Europe. 

She will represent the USA team in the 2019 IAAF World Outdoor Championships.

This book is a take on the Roman historians Plutarch (’Ploo-tark’)  “Lives” where he compares and contrasts different Greek and Roman figures.

Cognitive recovery often occurs after physical recovery has happened

Some students do not feel comfortable with accommodations (504 plan)

A 504 plan is NOT necessary in every case. Accommodations are meant for only short term for recovery

Not every physician is educated in academic accommodations, parents need to ask.

It is helpful to have the Concussion injury Coordinator to facilitate (CIC)

Case study:

A full day of school is key in the early stage of recovery, may cause the return of symptoms, even though at rest and previous days stated NO symptoms. The cognitive load of school also helps in concussion evaluation.

Physical symptoms- Headache, Nausea, dizziness, balance issues, blurred vision, light and noise sensitivity, sleep deprivation.

Cognitive symptoms – Concentration,fogginess,recall, mental fatigue, slow processing

Emotional symptoms – irritability, anxiety, sadness.

Pair a symptom with an intervention/accommodation.

Visit with physicians, provide resources for sample academic accommodations, present to physicians.

More about Dr. Summer Ott

Dr. Summer Ott is an Assistant Professor in the Department of Orthopedic Surgery at McGovern Medical School at The University of Texas Health Science Center at Houston (UTHealth). She also serves as the director of the Concussion Program at Memorial Hermann Ironman Sports Medicine Institute.

Dr. Ott is a Neuropsychologist specializing in sports-related concussions. She has extensive clinical and research experience with sports-related concussions. She currently serves as the team neuropsychological consultant for the Houston Texans and Houston Rockets, as well as many universities and school districts in Texas.  Previously, Dr. Ott served as a neuropsychology consultant for the Houston Dynamo and the Houston Aeros.

Her research efforts focus on proper baseline testing, concussion in female athletes, effects of physical exertion on cognition, and long-term consequences of repeated head injury in athletes. Dr. Ott has co‐authored publications on neuropsychological assessment in scholarly journals and frequently conducts presentations regarding the proper management of concussion.

I recently interviewed an NFL AT and the biggest concern was life balance and raising a family.

Alisha and Kevin King are both life balance people. We try not to use work/ life balance because work is part of your life that needs to stay in balance. trying to completely separate them causes more problems.

What is the best practice for using topicals and Wound Care?

The NATA position statement on wound care is that soap and water are equally effective as other more expensive and less readily available options for wound cleansing.

When we are on the field flushing with soap and water might not be the best option…because honestly, I do not want the bloody elbow close to my water bottle or water nozzle.

We all know that one athlete that might keep their wound back and not contaminate the rest of our water…but that one is generally the exception.

What is wound care best practice?

  • Clean the wound
  • Clean the area around the wound
  • Use an active wound healing agent like Hypochlorous acid
  • Cover with an occlusive dressing
  • Change the dressing and reapply the stabilized hypochlorous acid daily
    • visually inspect the wound and surrounding area during changing of bandage
  • Continue until wound is completely closed

Want to hear a strange Wound Care story?

Molly Quigley shared with us that she used the lining of an eggshell to help seal a wound.

Obviously we needed details:

Okay, so I had an athlete bite his lip during a basketball warm-up and we tried everything to get the bleeding to stop but after a whole quarter, the ref kept sending him out. I had run out of ideas but remembered reading a story about how the lining of an egg, specifically the little gross white lining you can see when you crack a raw egg, and how it can help to seal/clot a wound. We were desperate so I stole an egg from culinary and cleaned it really well! I got a tiny piece of the lining and put it over the wound. We gave it about a minute to dry and it actually formed an almost dry layer of skin that totally stopped the bleeding! He pulled it off after the game and it was fine, could barely see a mark and left it clean!

Are we overusing triple antibiotics?

Jason Carruth and Morgan Rakestraw reported that neomycin, 1 antibiotic in triple antibiotic was recently shown to reduce granulation and delay healing. Also, up to 25 % of people have an allergic reaction to it. Instead, use vaseline and maybe polybiotic if the wound is angry

What holistic approaches to wound care?

*****use caution as some of these are unregulated in manufacturing*****

Daniel Young shares from his own experiences:

  • Burns – lavender oil is magic
  • Cuts/abrasions/general wounds – tea tree oil layered with lavender oil does wonders.
  • I'll encourage kids/parents to try it. These oils fairly cheap and we buy them at Wal-Mart/HEB/Target. But, we do not use it at the HS.

PLEASE DO NOT…

Use Alcohol on an open wound. Alcohol is great for killing living things, but it does not filter through what it kills. Alcohol will kill the good cells as well as any possible bacteria.

Use caution with peroxide as it is a less cytotoxic solution, but it still kills healthy cells.

Hydrogen peroxide is a great way to get the dirt out of a cut that can not be washed out with soap and water.

How do we Care For The Active Wrist

One of the huge tips she continued to share was REFER ANYTHING YOU REDUCE!!!

I have reduced injuries before and since meeting Dr. Teunis I always refer them to get an x-ray before allowing them to return to play.

Mallet Finger:

What is it: Extensor tendon rupture or avulsion involving DP joint.

Treatment– Splint religiously! 6-10 weeks

Alumaform vs. Stax Splint- Not all 1 size fits all. consult with your local hand doctor to see what they prefer and why. discuss ways to ensure patient compliance and seamless transition from office to AT Facility.

NOT ALL Mallet Fingers can be splinted, some require surgical intervention.

Jersey Finger-

What is it – Flexed finger pulled into extension (commonly ring finger, 75%) rupturing flexor tendon.

Treatment:

Dislocations

Dorsal Dislocation-  splint 1 week, then buddy tape

Volar dislocation – much less common, extensor mechanism disruption, hard to reduce

Mimickers-

  • Traumatic Boutonniere
  • Skiers Thumb

Scaphoid Fractures

  • Hard to treat
  • hard to diagnose.
  • Common in football – “see these in December”

Scaphoid fractures…about 40% are missed and that is bad news, they take several months to heal due to poor blood flow. if it dies you get arthritis in your 20s.

What is Third Party Billing?

Third Party Billing means a patient comes in to see the AT and the AT sends the bill for payment to the insurance company. It does not mean you are working at a clinic and being paid hourly by the general fund, but you are billing for services as a PT or DC would.

Who is Joe Greene?

Started at the University of Wisconsin Hospital in 1991

Through various roles at the hospital, he was able to quickly move into an administrative role. Coupled with his state leadership roles, He was really able to look at Third Party Billing a make an impact.

2001 Implemented Autonomous Billing in Wisconsin

What is Orthovise?

The company Joe started to work with physicians and continue growing the autonomous billing aspect of Athletic Training.

2016 NATA issued an RFP pilot project to Joe Green and his team as the pioneer in 3rd party billing

What are the goals of the third party reimbursement project?

Ensure ATs receive reimbursement by insurance companies consistent with other allied healthcare providers. If a PT does manual therapy billed at $60/hour, then the AT acting within their scope doing the same task can be reimbursed at $60/hour.

Secure Consistent recognition of Athletic Trainers as allied health care professionals

What has the pilot study revealed?

  • When ATs bill according to the policy they are reimbursed at a consistent level
  • There are some gaps in analysis
  • Are AT as cost-effective?

What Education is going on with this?

Payor (consumer) and Employers (hospitals/physician) are both needing education

Opioid Abuse in Athletics – 580

Ever experienced Opioid Abuse?

Dr. Obie Benavides lost a student-athlete to opioid abuse that was an inspiration to him.

One single pill that was not from a pharmacy but a “friend” was all it took to end his life in March of 2018.

What Opioid Abuse?

Opioids / Opiates are similar but not identical.

Opioids are synthetic chemicals whereas opiates are plant-based or naturally occurring from opium

Using them other than as prescribed is opioid abuse.

  • Using them for longer than prescribed
  • Giving them to someone else
  • Hoarding them for later use
  • Selling them

What does it LOOK like?

Obie says some of the signs and symptoms of opioid abuse might present as:

  • Pupils being off
  • Constipation
  • Nausea
  • Requestion medication by name and dosage
  • Being absent
  • withdrawing
  • Itching
  • Dry Mouth
  • Low Tetosterone levels

What can we do as Athletic Trainers?

Patient Education is number one.

  • Develop a Post-op Pain Management Plan
    • There will be some level of pain
    • Medications will help manage the pain
    • Use pills as needed for this surgery
    • Properly dispose of unused pills
    • Do not share unused pills with anyone
  • Start the dialogue with the athlete before the surgery
  • Involve the parents
  • Educate Parents
  • Establish Real Pain Management Goals
  • Monitor the athletes
    • Attend surgeries when possible
  • Log the procedure and medications prescribed
  • Follow up within a week and document number of times meds have been used

Be willing to have uncomfortable conversations

More about Dr. Ifedi

After graduating magna cum laude with his Bachelor of Science in Biochemistry from the University of Houston, Dr. Benedict Ifedi received his medical degree from Baylor College of Medicine in Houston. He went on to complete his family medicine residency at Memorial Family Medicine Residency in Sugar Land, Texas, where he also served as chief resident. Afterwards he completed a fellowship in sports medicine at Bayfront Health in St. Petersburg, Florida.

Board certified in family medicine and sports medicine, Dr. Ifedi treats patients ages 5 and older. He provides care for sports-related injuries, such as strains, pulls and concussions, as well as wellness and preventive medicine. His clinical interests include management of chronic diseases, exercise medicine and physical activity.

He is an active member of the American Academy of Family Physicians, American Medical Society for Sports Medicine, and American College of Sports Medicine. Patients in Katy and the surrounding communities enjoy his welcoming smile and genuine concern for their well-being. Dr. Ifedi’s goal is to help his patients get back to feeling their best and to help teach them the best methods to achieve optimal health.

In his spare time, he enjoys traveling as well as watching and playing sports.
