Diagnostic Accuracy – 635

Do you personally have a Diagnostic Accuracy cutoff in terms of diagnostic factors to determine if you would use the test?

No, but if there are multiple tests available I look at the most accurate ones. I do not really like it when clinicians do every test available just because.

Are we image happy?

X-ray, MRI, Ultrasound – it is visual, so it gives us something to see

But blood tests do not really give us that.  It has to be done against controls.

Having every injury get an MRI would be great, but it is cost-prohibitive for a lot of people

“If we are sending every patient to get an ultrasound image, then why are we there?”

Talk about treating the patient vs treating the image or diagnostic test.

  • In Athletics, the nice thing is most injuries are life-threatening.
  • But for internal injuries MRIs are lifesavers.

It is hard to argue with an x-ray showing a fracture

What is the conversation you would have with parents before the season begins about your confidence in your evaluations and role at the school?

If I think we can treat them here we will if I think they need imaging I will refer them to our team doctor and he can order the images.

Mention this though: “a non-sports medicine doctor will likely default to out two weeks”  once they do that we are obligated to follow that before beginning rehab and return to play work.

Is there any uncertainty in using diagnostic accuracy values in Athletic Training?

YES!  There is untold value – the new ATs have learned this but it is a new development in AT education.

How do we make Diagnostic Accuracy Language more universal?

It is growing as we have more and more ATs learning about it as part of their education.

When would you not focus on diagnostic accuracy measures?

In short, yes you can go with your gut and try a few other tests when things seem off.

How do you get students to buy-in?

This is how you win arguments.

If someone questions you and your evaluation you Have numbers and research to back it up…this usually will end the argument.

Make them understand it is not just statistics.

Some orthopedic tests have minimum research or no reported value, where does proof of the value come in?

Yes, you can still do the test if there is a value that is not reported.

Usually, this requires MRI proof.

Dr. Kathy Nguyen is a primary care/sports medicine physician.  She is a team physician for multiple high schools, helps with colleges in the Houston area and is the team physician for the Houston Dash soccer team.

Do you know what is considered best practice and recommended by research?

As athletic trainers, we routinely see a variety of skin issues happening with our athletes.

Blisters: do you pop it or not?

Today on the podcast Dr. Nguyen relays to us the best practice in issues as common as blisters and as serious as tinea issues that may be resistant to topical antibiotics.

At minute 2:10 on the podcast, Dr. Nguyen says she always recommends leaving the blister if possible.

Around 4 minutes on “Skin with Kathy Nguyen”, she discusses wound care specialist opinion on new skin growth and scarring.

What are truths and what are myths?

Through research and discussion with other wound care specialists, she helps us sift through what are considered old wives tales and gives us concrete reasons to refer a skin issue or lesion to a doctor.

Masters of Athletic Training Year 1- a look back -148

We are broadcasting live and interviewing the speakers of the Memorial Hermann's Sports Medicine Update.  Some of the recordings pick up noise, but this is a live event.

External Bracing with Chris Dean and Maurice Hawkins – 115

Maurice Hawkins from Gazelle Health Care lends us some expert opinion on the topic of external bracing focusing specifically on the Ankle Roll Guard invented by Chris Dean.  The Ankle Roll Guard fits outside the shoe and acts as a block for inversion ankle sprains.  we do have concerns about this product on football or soccer players, but Chris says he is planning to do some trials with outside sports, but for now he has been focused on court sports since he is an avid basketball player.

John Harmon and I have spent a good bit of time together riding to meetings for GHATS.  One time we began discussing things we wish they had taught us in college to help us succeed as an athletic trainer in the secondary setting.  Budgeting, building relationships with vendors, insurance claim forms, student athletic trainer programs…they may have taught us something about them, but we could not recall lessons or courses on these topics.  So we decided to share some stories and things we have learned.  Nicole Cascia is going to be a rock star after hearing from so many experienced ATs about how to not fail.
