Nancy Burke – Women of AT – 537

A pioneer, innovator and hard worker, Nancy Burke has never been afraid of challenging the norms.

Her induction into the NATA Hall of Fame symbolizes how much change one person can make.

Nancy Burke, NancyBurke, Hall Of Fame, NATA Hall of Fame, Public Safety Athletic Trainers Society

Who is Nancy Burke?

  • Career spanning 4 decades
  • Certified in 1975
  • Eastern Kentucky Graduate Assistant
  • NATA Hall of Fame inductee
  • Pioneer
  • Innovator
  • Fairfax county Athletic Trainer for 28 years
  • Started Public Safety Athletic Trainer Society
  • 7 siblings with lots of nieces and nephews

What does retirement after 40 years of AT look like?

Nancy says she…

Played golf

spent some time on her pontoon boat

but that led to…boredom

How does Nancy Burke continue to grow as an AT?

  • Surround myself with forward thinking people. ATs who ask “What will Athletic Training look like in 10 years, 20 years?”
  • Getting her NPI number and advocating all ATs to get theirs.
  • AT Journals and publications
  • Reading up on various State Practice Acts
  • Building relationships with local ATs
  • Attend state and national meetings
  • Collaborate with both older and younger ATs

What are Nancy Burke's top moments in Athletic Training?

Receiving the phone call to be inducted into the NATA Hall of Fame.

Building lasting relationships with students / parents

Getting a phone call from Marjorie “Marje” Albohm requesting that Nancy be a speaker for the national meeting.

Discuss a low point in your career?

Working as the GA for Eastern Kentucky Nancy Burke was expected to be the next hire for the female At position.

She was not hired for that position

This made Nancy questions who she was, what she wanted to do and why she wanted to do it.

Ultimately after a few days in hiking in the Blue Ridge Mountains she figured out she wanted to make an impact at the high school level and with 4 months that is where she was working.

Becoming a Physician Assistant – 530

How can an AT become a PA?

Interested in going from AT to Physician Assistant?

Reagan Dunnahoo has a unique skill set.  She was a practicing AT who went back to school to get her Physician Assistant license.  Reagan is now working at a small school where she is both the PA and AT.

PA, Physician Assistant, Physicians assistant, AT To PA, Reagan Dunnahoo, Hatch Schools

What is a PA?

Here is an infogrgraphic from AAPA.ORG

PAs are medical professionals who diagnose illness, develop and manage treatment plans, prescribe medications, and often serve as a patient’s principal healthcare provider. With thousands of hours of medical training, PAs are versatile and collaborative.

PAs practice in every state and in every medical setting and specialty, improving healthcare access and quality.

Who do the skills of an AT and PA work together?

When Reagan went to PA school she felt she could have done a better job leading the orthopedic section of the course than her instructor due to her extensive training with AT.

As a PA you have a greater responsibility for each patient and your patient population is larger.

Skills learned as an AT absolutely help Reagan succeed as a Physician Assistant.

Things to really consider:

PA school consumed about 95% of her time.

Over half of her graduating class is not still practicing as a PA

You are not allowed to work to earn income while attending most programs

Only one marriage survived her cohort

There is a lack of autonomy with PAs. It is actually easier to work on your own as a nurse practitioner than a Physician Assistant

How can I ask more questions of Reagan?

DermoNueroModulation – 520

I do not even know what it is…

Brett McQueen loves Athletic Training and nerdy stuff.  He suggested we talk about Dermonueromodulation.  I am learning right along with you.

What is Dermonueromodulation?

Well using my medical terminology i believe Dermonueromodulation goes like this:
Dermo – Skin
Nuero – Nerve
Modulation – Change

But beyond that I am really at a loss.

Building a Sports Medicine TEAM – 502

Who is on YOUR Team?

Are you and local medical providers on the Same Team?

Dr. Josh Yellen is the program director for the University of Houston MAT program.  He must build relationships with local medical providers to give his students the best possible exposure.  He is teaching his students to be on the same team and build each other up as we grow in the medical profession.

What are you doing?

What have you done that has worked in building relationships with local medical providers?

Have you gone out and met any of our local providers?

Have you brought other medical professionals into your Athletic Training Clinic so they can see what you do or even co-treat athletes?

Tweet it to me or comment on the live Facebook Live Video

Here is what we recommend:

  • Create a one page letter to give to local medical providers
  • Ask to meet with the office manager
  • Ask what their rehab protocols are for certain injuries
  • Invite them to join you for  day in the clinic or sidelines
  • Attend workshops outside of your normal domain
  • Set up a training for equipment removal and invite the local EMS and standalone ER clinics

Have a business Idea?

Athletic Training needs people who are willing to challenge the norm.  At Innovate AT you can join a group of like minded ATs who are trying something new.

Dr. Yellen said we do not understand the business of medicine… so with Innovate AT we are trying to be in business.

Building Relationships: Local ER – 497

Do you know your Local ER Doctor?

Here in Houston we have a HUGE medical center and tons of small ER clinics and facilities.  There is likely no way I could meet and know each ER Doctor in my area.

ER Doctor, ERDoctor, Emergency medicine, Gail Margerum, Doctor Gail, Dr. Gail Dr. Gail Margerum is a fan of the Sports Medicine Broadcast and and reached out to share some knowledge after an episode where someone discussed a blown retina.

Dr. Gail did not know much about Athletic Trainers until she started listening to the show.  Now she is ready and waiting to build relationships with ATs.

But I should know more of them and should really get to know the office manager and create and ED liaison in the event that we need to send kids there.

I have not been very good about that historically.

What can you do?

Email me some of the things you have done or are doing to build the relationship with your local medical providers.

I want to set up a schedule to where every …say Monday I am out of the school visiting local Medical facilities.  There is so much good in collaborating as medical professional.

Adam Halpern of Innovate AT recently shared this article about working together:

https://www.score.org/resource/solopreneur-series-networking-your-business?fbclid=IwAR0XUtBnR_zuIyihbjC3nwYi2DNo_Seo07_p7aPD4HkK9thTxIscsQC9m7k

What other resources do you need?

Dr. Gail Margerum shared a great list of resources to help an ER Doctor and Athletic Trainer be on the same page.

Free ap: MD Calc that has following useful trauma rules/guidelines: Canadian C spine rules for who might need xray

Canadian CT head rule for who needs CAT scan

Nexus C-spine rule: for who might need neck xray

Ottawa Ankle Rule: who needs ankle xray

Ottawa Knee rule: who needs knee xray

Pediatric Asthma score: who will need to go to ER for treatment

PECARN Pediatric Head injury rule: what children need head CAT scan

                            GLasgow coma score

FP Notebook App: guideline for almost any medical conditions: wasp stings, allergic reactions, bruises, heat illness, etc.

OrthoFlow app: touch the part of body injured/fractured and it tells you how to treat it

Need More advice?

Justin Streiff send a few recommendations based on his current situation and success

1- One page paper of newest concussion laws/rules and ER does not clear concussion.

  • Just refers to sports/department that handles concussions.
  • Also on this is the need for a note with specific guidelines lines for the patient.

2 – Have a call in number for anytime you are sending a kid to ER.

  • They add our concerns and contact information to the notes for the Dr. to see.
  • Also add to return supplies to parents or hold and I will pick up.

3 – Supplies damaged/ not returned: My department billed the ER a few times and this got the message out. This is what the local ambulance service does as well.

4 – Sports medicine split a consultant with the ED. This has helped bridge the gap greatly.

5 – All athletic trainers on staff do a 3-day observation  annually in the ER.

6 – We do equipment removal and spine boarding with as many residents as possible before football season as many of these residents will assist in covering local football games with the ATCs.

7- Education of PARENTS:

  • what the ER is and that subsequent specialty visits may be needed.
  • Also empower the parents with information and specific questions to ask the Dr.

Shoulder Evaluation Tips and Tricks – 492

Shoulder Evaluation can be hard.

I need help with my shoulder evaluation skills.  I have plenty of room to improve.

Last podcast focused on Shoulder Anatomy building on that we are looking at evaluation tips and tricks to help me (and anyone else) refresh and grow my skills.

One thing we must remember is that a shoulder problem can really be a hip issues and the lack of mobility is stressing the shoulder more.

Some of the tips covered on this podcast:

  • Tendons – painful at warm up, loosens up, but then painful after stopping
  • Labrum – Pain throughout the activity, decreased accuracy, clicking sound or feeling
  • Impingement – anterior pain with specific arm placement
  • Quick isometric in each direction: Cyriax Selective Tissue Tensioning for contractile vs non-contractile tissue differential dx
  • Contractile = painful with contraction and stretch
  • Non-contractile – no real pain with stretch but pain as you passively move into contractile direction

Shoulder Anatomy – 489

Are you a Shoulder Anatomy expert?

I am not a Shoulder Anatomy expert.  I know there are things I can still earn and use to help me understand, evaluate and rehab the shoulder.  When I do not know an answer or notice a consistent weakness I normally head towards gaining understanding.

Fortunately I have some friends and colleagues that can help me gain understanding.

Bubba has 3 shoulder surgeries under his belt and needs a fourth.

Marcus Knox, a Physical Therapist for Houston Methodist, joins the podcast for the first time.

What is this podcast about?

Always improving the profession of athletic training…but if you are this far into it you probably mean this specific one.

Here we are focusing on shoulder anatomy to review, identify some tips and discuss some unusual anatomy findings of the shoulder.
