Cramping Neurology – 573

In Cramping Science interview #1 with Dr. Kevin Miller, we did not even get to Cramping Neurology. We spent about 90 packed minutes discussing your questions and the role hydration plays in cramping.

Dr. Miller and Mike McKenney knew there was no way to do justice to cramping science without addressing the neurology component.

Cramp – an involuntary painful contraction of skeletal muscle brought on by a variety of factors. This podcast focuses on cramps related to the exercise of healthy individuals.

There is a teeter-totter between excitation and inhibition of the muscles from the nerves.

Cramping Neurology, nerve cell, axon, dendrite, Dr. Kevin Miller

What is Cramping Neurology?

Most basic definition: the role of the nervous system in cramping

About 8 minutes in Dr. Miller discusses the 3 main nerve cramp theories:

Central Theory –

This is the best supported through research stating the cramp occurs at the nerve cell body

There are two threshold levels for the cell body to cause a contraction known as a “bipolar state.”

Peripheral Theory –

Happens along the axon or end of the motor neuron.

We can stop a cramp from occurring by blocking the afferent activity with a nerve block to the tendon…but it is not always true.

Muscle Fiber Level Theory –

Least favorable among researchers but says that it happens at the muscle fiber level

Does the brain adjust and make the cramp easier?

Around minute 15 Mike asks if cramps can become a learned response from the brain

Of course, Dr. Kevin Miller has an answer that is helpful but not definitive

He does remind us:

  • each cramp is unique
  • there are many factors
  • frequent crampers need to know their triggers

Dr. Sandra Fowkes Godek knows cramping science. If you go to her bio on Levelen you will see:

“Dr. Sandra Fowkes Godek is a world-renowned expert in Hydration Sports Science, with over a decade of research in thermoregulation, athlete sweat testing, and fluid & electrolyte replenishment. “

Sandra Fowkes-Godek; Fowkes; Godek; Cramping Science Research

Her West Chester University bio shows even more.

She is the director of the H.E.A.T. Institute

Truly if you want to nerdy out about cramping science read some of her research or find a way to connect with her.

So what about Cramping Science?

Man oh man oh man.

Dr. Fowkes-Godek is smart, quick and a great talker. the numbers and data she can just rattle of about cramping, chloride, potassium, sodium blood levels is amazing.

But she is fun to listen to and engaging.

What is even more she is coming back on the Sports Medicine Broadcast to continue discussing cramping science.

Some of her research with the Philadelphia Eagles included taking guys who were in active full body cramps and drawing blood. How's that for suffering for science.

She also spent hours making specialized drinks to help athletes replace what they lost.

If you can shake dried sweat off your shirt, then you are a salty sweater and need a little extra salt (without added potassium).

What about heat illness and cramping?

If we train people to drink when they are thirsty and give them free access to water we can almost assuredly avoid dehydration.

This does not mean heat exhaustion is only related to hydration

Isotonic Drinks – Cramping Science – 571

Isotonic Drink – neutral or balanced. Fluids and electrolytes can move freely across the abdominal walls without requiring extra processing

Hypertonic Drink – formulas more dense than normal blood causing the body to pull water into the abdomen to process

Hypotonic Drink – are lower in electrolytes than the blood creating a movement of carbohydrates and electrolytes into the stomach and out of the blood

What are some examples of each?

Isotonic – Hoist Hydration (use coupon code “TheSMB”)

Hypertonic – Pickle Juice, The Right Stuff.

Hypotonic – Powerade, Gatorade

What are Mike McKenney's Thoughts?

Mike previously joined the Sports Medicine Broadcast to discuss hypo-hydration and some of the research he has been doing.

As more of an expert in cramping science Mike is leading the series

When are Isotonic Drinks worth drinking?

HOIST was actually created by some hard partying college grads as a way to “cure” a hangover. So if you are going for a night on the town take an Isotonic Drink with you to finish off the night and improve your morning.

Dr. Mark knoblauch discussed how living a low sodium diet has helped him live with greatly reduced symptoms of his Menier's and BPVD.

If you are not sweating and burning electrolytes and calories or have a need for low sodium in your life seek medical advice first.

Preventative Data from Preventicx – 569

Kevin Robel joins Dr. Layci Harrison to discuss Preventative Data Analysis.

Kevin Robel; Layci J Harrison; Preventicx; Preventative Data

Have you been in this situation?

You started collecting data on injuries, doctors visits, attendance, and all of these other random things…

Every time some one on Facebook posts about showing your value you get excited…

That turns to frustration due to the inability to do something with the data.

https://www.facebook.com/sportsmedicinebroadcast/videos/325442741654381/

I hate data analysis

It doesn't matter if it is preventative data analysis…

Injury data…

or anything set of numbers that you have to look at and draw conclusions from.

Kevin Robel discusses how we as Athletic Trainers often do not collect data because we are so busy.

What is Preventicx?

A data analysis consultant that helps you move all of your data into a centralized cloud based storage location.

Kevin can help you visualize your data in a way that is easy for the average person to understand.

The future is being given to data analyst

Have you read Moneyball or or Astroball?

The managers of those teams made huge decisions based on data analytics.

Ask any administrator about getting a new person and they will ask you for your data to support the need and the value it would provide to the program.

New administrative jobs are being given to people who can collect, interpret and produce visuals based on data

Sports Performance Coach – 568

Jerade Parks is an Athletic Trainer, Sports Performance Coach, life coach, conference host and aspiring public speaker.

That is a whole lot for one person to handle.

For Jerade it is not all fun and games…. He almost lost his car chasing this dream.

Bit of a free Spirit?

Jerade admits he is more free spirited, artsy and very talkative.

He is creating his own space in our ever changing profession. He has experience in a multitude of rehabilitation to performance settings.

For example:

  • athletic training
  • rehabilitation
  • reconditioning
  • fitness
  • sports performance/ athletic development
  • sport coaching
  • recovery
  • nutrition lifestyle-based nutrition coaching

Who are the Sports Performance Coach clients?

This includes treating, training, and coaching general population, weekend warriors, and youth to professional athletics

What more can you have your hands in?

As a somewhat selfish endeavor he began hosting CEU conferences to obtain the certifications or training for free while still getting all of his CEUs in.

This turned out to be a great way for him to build his network, provide a valuable service to Athletic Trainers and related healthcare professionals.

Hire an Expert

Adam Halpern of Innovate AT discusses his recent hiring of a virtual assistant.

Know what you are good at and what you are not. If your talent is being in front of people then hire someone to do the work that keeps you behind a computer.

Jerade hired a business coach

I reached out to a former student of mine about helping compose emails to each guest after the interview and after the episode releases for them to share their interview.

****Update**** My former student Jazmin has been scheduling and send emails for about month and it has been fantastic to have that follow up. Lots of guests have responded which creates a dialog and future opportunities. It was something i wanted but could not manage with everything else.

Publishing a Book – 558

Dr. Mark Knoblauch and Dr. Jeff Konin join forces to teach us about publishing a book.

I am an AT, I Can't Publish a Book

This is pretty much the statement I said to Dr. Knoblauch when he suggested the topic.

But then he mentioned our adoption story. we now have experience with the system, emotions, training, rejections.

Sometimes publishing a book is only to have your story written down.

Others it is to entertain someone

Furthermore others are written for education.

Some of the discussion topics:

  • Book idea conception and design (when/how to turn a thought into a book)
  • Publishing process
  • Getting a publisher and/or agent
  • What is involved (writing, editing, formatting, cover design, marketing, obtaining reviews)
  • Expenses involved for self-publishing (cover design, ISBN, etc.)

Why Dr. Konin and Knoblauch?

Dr. Konin has a little experience…

  • Author of 24 published textbooks, actively working on writing 3 textbooks now
  • Books have been translated in Spanish, Portuguese, and Korean
  • Special Tests for Orthopedic Examination in its 5th Edition, Allied Health Best Seller first published in 1997
  • Author of 30 textbook chapters
  • Has published for Slack, Human Kinetics, Elsevier, and self-published

Dr. Knoblauch owns a publishing company

As previously discussed on Imbalances and Disequilibrium, Dr. Mark Knoblauch owns a small publishing company.

  • Certified as an athletic trainer since 1996, as CSCS since 1997, and licensed as LAT since 1999.
  • PhD in Kinesiology, Post-doc in Molecular Physiology and Biophysics 2012
  • Ten published books, several in development
  • Published book topics include vestibular disorders, special diets, running, time management
  • Publishing includes both self-published (print-on-demand) and direct-to-publisher

Tanya Watson – 540

Tanya Watson actually lived about 5 minutes from where I live now/ Although she has moved to New Mexico and spent most of her recent career there, we are connected.

The Women of Athletic Training

What is your AT Story?

Tanya Watson grew up in Las Cruces NM and found out about Athletic Training in Middle School and went to the Athletic Training Clinic at New Mexico State University (NMSU) for “growing pains” in her knees.

Tanya attended NMSU, graduated in 2001 from the Athletic Training Education Program (ATEP) and BOC Certified in April 2002.

She then worked as a Licensed Athletic Training  Intern at NMSU for the Spring of 2002 covering the Track & Field team.

Following NMSU Tanya attended graduate school at the University of Wyoming 2002-2004 & obtained an MS in Kinesiology. While there she worked in athletics with football, swimming and diving, track and field, and wrestling.

Tanya was an Assistant Athletic Trainer at University of Alabama Birmingham from August 2004-June 2006 working with Women’s Basketball. She also supervised GA’s working with women’s sports.

From July 2006- July 2009 she was an Associate Athletic Trainer at The University of Southern Mississippi in Hattiesburg MS, Working with Women’s Basketball & Men’s Tennis.

Tanya Watson was also adjunct faculty and the Interim Clinical Coordinator for the ATEP (2007-2008).

She returned home to Las Cruces New Mexico in August 2009 and worked at Las Cruces Orthopaedic Associates as a Physician Extender and Clinic Manager until September 2014 when she accepted an emergency hire Term/Temp Faculty position at NMSU to teach in the ATEP.

May 2017 earned an MA in Curriculum and Instruction at NMSU. Now I am the Athletic Trainer as Las Cruces High School (my alma mater).

What leadership are you giving to ATs now?

Tanya Watson is serving in her current role as Southern Rep on the New Mexico Athletic Trainers Association Board. She has been in this position since 2016.

What are some of your best memories in AT?

As a woman in AT what are some of your challenges?

Coming out of college Tanya was not even considered for professional sports jobs.

Felt an internal pressure to put her career on the back burned to move home and be closer to elderly parents. Men do this as well, but more often it is women taking care of the family over career.

What do you want your legacy to be?

That every athlete I took care of knows they received my best care I was able to provide at that time. That their skill level or nationality did not influence how I provided care.
