Welcome to AT Leadership

In the book Only One You life lessons abound.

“Always be on the lookout for a new friend.”

John and Jeremy have no connections 4 years ago. Just social media friends.

We have grown to be trusted friends. I would let John stay at my house for a week if he was in Houston.

I have begun texting a group of guys each week so that I am being intentional about building relationships. I schedule that text early in the week so that I know it is going out.

“Look for beauty wherever you are, and keep the memory of it with you.”

For John, this page of Only One You reminds me to keep the memory of the good moments or wins or championships for when times are tough (tough teams, injuries, coaches, etc.)

This line speaks to me to not take pictures of everything, but be in the moment and take a picture in my head. create the moment instead of focusing on the perfect shot

John: Leadership Capital – Know when to speak up but know when you sit back and listen. For me, those that stand out have egos. This never helps with relationship building.

Jeremy: When I started in 2012 there was maybe one other sports medicine podcast focused on Athletic Trainers. I did not sit back and wait for someone else to start one. From October 2013 through October 2019 there have been over 524,00 downloads of the Sports Medicine Broadcast.

“Know when to speak; know when to listen.”

This goes back to “blending in” Speaking is important but only after you have all the information. Listening is the most important aspect of leadership.

Mark Knoblauch told me: “The ATs who get angry and get into arguments are the ones who are usually inexperienced and have a need to prove themselves.”

Or there is the Fight Club quote: “Most people do not really listen they only wait for their turn to speak.”

“No matter how you look at it, there is so much to discover.”

Both John and Jeremy agree “One of the things I’ve learned doing this podcast is that; there is always more to learn. Every time I think I've figured things out, I realize there’s something else I need to know.

“If you make a wrong turn, circle back.”

John likes this quote from Only One You. He knows mistakes happen in our daily and professional life. Leat's learn from them and “circle back.”

Repeat podcast guest Brandy Currie says reflection is key and she does it daily.

Then there was the time my son dropped a googly eye into his ear…our circling back was to buy an otoscope.

“If something gets in your way, move around it.”

Jeremy: Usually when I am given a rule my first thought is how can I get around this… not always the best way of thinking, but I tend to look for other options.

John: Realize things change, schedules change, obstacles will occur but relax, detach, adapt and overcome.

This is another opportunity to reflect and circle back on how things can change and listen to all of the details before speaking.

“Set aside some quiet time to relax and reflect every day.”

John: This is one of the most important things to me in that every day I get time to myself. Most of the time I have to make it whether that’s getting up at 4am to work out or scheduling time on my calendar to actually sit and read. We also covered this with “Learn to Say No” and Prioritize and Execute.

John LOVES to read, so sometimes he wakes up early, goes to the gym and rides the bike so he can sit and read

“Appreciate art. It’s all around you!”

Have fun during games, enjoy the small things and enjoy when your athletes achieve their goals.

“Make wishes on the stars in the nighttime sky.”

John: Have a vision, set goals create your value.

Jeremy: Take time to look up and see the stars.  The small things that are always there. Do not look past what is in front of you.

My wife does this with our youngest…just sits back and watches.

I often ask my family to not say I can not wait until “_______” is over. there is way too much to miss.

Adri did a backward somersault and smile. He was excited to go out into the world with what he had just learned. “‘Wait for me!” he shouted to his friends.

Dr. Kathy Nguyen is a primary care/sports medicine physician.  She is a team physician for multiple high schools, helps with colleges in the Houston area and is the team physician for the Houston Dash soccer team.

As athletic trainers, we routinely see a variety of skin issues happening with our athletes.

Blisters: do you pop it or not?

Today on the podcast Dr. Nguyen relays to us the best practice in issues as common as blisters and as serious as tinea issues that may be resistant to topical antibiotics.

Leadership Myths – 595

“Today, when leadership is discussed, we usually purport to be discussing what leaders do, rather than what they say, as if this were a more clear-eyed approach. The truth is that when we look closely, we see leadership as much in what our leaders symbolize as in what they accomplish. 

Whatever the case, our obsession with leadership seems to be constant, if not growing.

The mythology of leadership, however, will be a growing source of frustration if we don’t come to some more clear-eyed sense about why this is so and why it matters to us.”

What about Leadership Myths?

We wrongly believe that what happened in one leadership instance can be replicated in another. This leads to the three myths, which helps us navigate the understanding of leadership.

The first of the three is the “Formulaic Myth.”

  • Ignoring the reality that leadership is contextual, we feel that leaders stick to a strict checklist, leading to success.

The Second is The “Attribution Myth”

  • When we view leaders in hindsight we see that their vision or mission can far outweigh those that surround the leader themselves.

  • Delivering results is all that's required for positions of power and accolades

Is the “Formulaic Myth” is effective in leading others?

Neither John nor Tory believes this method produces good consistent leadership. Tory feels the opposite is true for him.

Leadership needs to be based on the people group and the topic.

Knowing your audience or followers is critical to the success of leaders. The leader is believed to be only marginally more important than the quality of the followers.

What is Leadership Culture?

Tory does not like to use the word “culture” because it is often not clearly defined. Instead, he talks about the environment you create around you.

It is essentially the same concept but with clearly defined actionable steps to improve and grow the team.

Define What You Value

Does your “culture” define what you value?

Does it reward and celebrate achievement?

Is it clear?

Is it scalable or reproducible?

Do the actions of your community continually remind you of your expectations and the environment you are creating?

Who are your heroes?

Are they real people?

This is a really hard question because nobody is perfect and to call them a hero we often have to overlook flaws.

One of my heroes in Athletic Training is Bob Marley. I have told him this more than once and he has joined me on the podcast multiple times.

Bob is kind and helpful, patient and well respected, well-connected and humble.

Then there was John Lineberger in college…there was always an adventure or story to be told with John. I made reference to this video clip during the podcast.

If you are labeling yourself that you are missing the tenants of that label.

A hero is a label good or bad – each person gets to give that label to each person.

It does not mean they are a good person.

Foot Injuries In Sports – 592

Foot Injuries in Sports are Increasing

30 – 40% of injuries seen in clinics are foot and ankle related

Dr. Taggart Gauvain shares some of his research and clinical knowledge with Shawn Ready, ATC LAT at the Memorial Hermann Sports Medicine Update

Some of the reasons we are seeing more foot injuries in sports:

  • Bigger athletes
  • Stronger athletes
  • The desire for lighter footwear to increase speed
  • Faster athletes
  • Increased mobility in footwear

However, Dr. Gauvain discusses the lack of research and evidence to confirm that lighter shoes are contributing to the growth in injury.

What are some of the troublesome foot and ankle injuries?

High-ankle sprain –

  • separation of the tibia and fibula.
  • We do not really get into the whole fix it after a week of rehab topic… but it is really controversial in the high school setting

Dr. Gauvain recommends the following for high ankle sprains:

  • Treat each one individually
  • adjust the plan to fit the need and injury
  • in general non-weight bearing in a cast or boot for 6-8 weeks
  • begin therapy for the mobility of joint
  • after mobility of joint returns begin working on agility and return to sport activity

Jones Fracture –

  • occurs at the base of the 5th metatarsal
  • non-union occurs about 10% of the time
  • Re-fracture rates are reported post-surgical intervention

Some have tried adjusting diet and increasing vitamin D

Plantar Plate / Turf Toe

  • Sesamoid fracture complicates things…
  • Some are born with bipartite bones
  • We lack great fixation techniques because of the size of the bone.
  • Mobility vs stability is a huge issue with plantar plate injuries. 
  • Finding the balance is always key

Lis Franc

  • Discuss the change in the mechanism of injury
  • Roma arch configuration gives a great understanding of the importance
  • orthotics may be partially to blame

How can ATs be more effective at collaborating with Doctors and diagnosing foot injuries?

  • Always discuss with an athlete the injury and what you think.
  • Tell them and write down what you would like to see
    • weight-bearing films are crucial
    • need bi-lateral comparison
  • Plantar Bruising
  • Dorsal Swelling
  • Inability to return to practice after rehab

Do we have a window for getting films on a midfoot sprain?

Refer to the above clinical signs as well as a few missed practices.

A missed diagnosis can create long term debilitating arthritis.

If there is a suspected Bone Bruise then Dr. Gauvain likes to refer for an MRI to ensure there is not something being missed

A quick Google search yields this definition: a person who is fanatical and uncompromising in pursuit of their religious, political, or other ideals.

“The moniker “zealot” can imply the erasure of conventional behavior, where anything is justified in support of the goal. 

Leaders who inspire others to seemingly irrational commitment are a special breed that can generate hurricane-like winds of disruption and change.”

How to spot Leadership Failures

Micromanaging

Old school – “That is the way it has always been done”

Not asking why things are done, just changing them to change them

Charging ahead with plans that seem to be flawed from the start

What do Leadership Failures teach us?

Obviously they can teach us what not to do.

With Zealots like Hitler, Robespierre, Al-Zarqawi bad things can happen very quickly. Like a very hot flame the quickly sucks up all of the oxygen in the room, burns really hot and then burns itself off quickly, zealots can cause a lot of damage quickly.

When we do not evaluate the whole picture and stop to think “Am I getting all of the information?” we put ourselves at risk of repeating these terrible leaders.

How do we weather those times of bad leaders?

It all comes back to knowing WHY you are here at this place at this time with these current abilities.

Know your resources available to you.

Escalated situations typically show more about the aggressor than about you. There is always something under the surface.

Kent Games Coaching is all about challenging the status quo. He was one of the youngest professors to be offered tenure. Kent was obtaining his dream job at his alma mater.

But that wasn't enough. He still felt empty.

Dr. Kent Games began evaluating his life through these questions:

Finding your why is as much about what it is is NOT as is it about what it IS

As professionals, we find there is not one cookie-cutter path, but as students, we are often lead to believe that A+B+C = D and that is how it should work.

The more you give your self permission to listen and explore that quiet whisper the louder your why will become. We are trained to tune it out and follow the leader.

Alisha kept pressing forward looking for that feeling of “I have arrived” but it was never there. She came to learn it is about the journey and not the destination.

Learning To Say NO – Kent Games – 589

Learning to say no can literally set you free. A friend of mine typically responds to requests with, “However I can help.” As Athletic Trainers, we are in the service business, so we tend to say YES to everything.

I was asked recently, “Jeremy, would your coaches fight for you to stay employed here?”

I am trying to find ways to be helpful and to build relationships without taking on someone else's job responsibilities. I am learning to say no. But am I learning to say no gracefully?

Learning to Say No

For many, leadership may imply the imposition to say yes.
Yes to new tasks. 
Yes to new projects. 
Yes to more jobs and gigs. 

Last year we learned with the Dichotomy of Leadership that a hard line on one topic can cause you to burn the candle at both ends, resulting in burnout.

Tribe of Mentors – Tim Ferriss

A Tim Ferriss podcast highlighted the “no” responses to his invitation to participate in his book Tribe of Mentors.

Tim asked hundreds of entertainers, athletes, CEOs, speakers and guests what makes them tick. 

His “no” responses were so graciously worded, that he put them in the book anyway! It was a way to highlight how some power brokers eloquently decline invitations to projects.

John reads a couple of these responses throughout the podcast.

What are some practical steps?

Dr. Games discusses the importance of acknowledging the invitation and showing gratitude, as evidenced in his automatic email reply.

John Ciecko also discusses how he recently took his email app off his phone to allow him to be more present.

For John and Dr. Games, this contributes to a reduced feeling of anxiety because he is not constantly checking and waiting for something to pop up.

“I can give you five minutes of my time right now. If you need more than that, you will need to come back in one hour (or when the practice rush is over).”

After hearing this, I discussed how we can implement it into our daily practice to better serve each athlete who trusts us with their care.

Why YOU should Learn To Say “No”

To give your attention and energy to the situations that deserve it.

If an opportunity does not line up with your “why” and your value system then gracefully say no.

Reducing Injury Risk – 588

Dr. Layci Harrison has a specialty in Movement Screens and their role in Reducing Injury Risk.

What is FMS training?

A Tool for identifying the potential for injury due to limitations in specific movements. It was developed by Gray Cook

Getting certified is a good idea and can really help you take advantage of the tests and results but it is not mandatory o use the tests to help identify potential issues.

What is the key to Reducing Injury Risk with FMS?

Protect before you correct

What tests does it involve?

  1. Deep Squat
  2. Hurdle Step
    • Tight Hip flexor may cause limitations in testing
  3. In-Line Lunge
  4. Active Straight leg raise
  5. Rotary Stability- probably the hardest one
  6. Shoulder Mobility- Hard to access
  7. Trunk Push up

Scoring the movement screens

0-pain in any movement
1- no pain but unable to do the movement
2- Complete movement but show some compensation
3- Complete movement no pain or compensation
A score of 14 or less equals a more likelihood of injury.
21 is a perfect FMS score

Dr. Harrison's Tips:

Use the score sheet from FMS, easy to find on Google.

It can help screen and individualize training.

Tai Chi or yoga may also help balance

  • Balance testing can be accomplished using BESS and YBT
    • BESS- Static balance
    • YBT- dynamic balance

A large part of reducing Injury Risk is knowing what to do with the data once we have it…but more on that another time.

More about Dr. Layci Harrison

My research interests include functional movement and how movement is related to balance ability. Specifically, I am interested in ways to improve functional movement and the effects of improved functional movement scores on injury risk.

I am also studying how the effects of balance training can be crossed from the trained limb to the contralateral limb following lower-body injuries as a way to expedite the injury rehabilitation process
